What to Bring to Your Appointment
To keep your health records current, we ask all patients — new and existing — to complete a fresh medical history form every 12 months. Please bring the following to your appointment (if applicable):
- Your current medications or a written medical history (we can photocopy these for our records)
- Information about any allergies, such as reactions to penicillin or latex
- Your health fund card or Medicare card
- Referral letters, previous dental records, or X-rays
We process all claims immediately through HICAPS for your convenience.

Child Dental Benefit Schedule
Looking after your child's dental health is a big responsibility. If you have a child aged 2 – 17 years, you may be issued a dental voucher from Medicare to the value of $1,132, and there are no out-of-pocket expenses if eligible.
*When booking appointments, you are required to pay a $50 booking fee.
